Welcome to Tiara Jones Counseling and Case Management Services! I'm Tiara Jones, a licensed clinical social worker (LCSW) with a passion for helping adults and families navigate life's challenges. With over 15 years of experience in the field, I've developed a reputation for providing compassionate and effective counseling and case management services.

My Journey

As a social worker, I've had the privilege of working with adults and seniors in various settings, including hospitals, community day centers, and private practice. I've had the opportunity to work with individuals, couples, and families, addressing a range of issues, from coping with trauma to managing chronic illnesses. My approach is rooted in empathy, understanding, and a commitment to empowering my clients to take control of their lives.

Service

As a case manager, I provide a range of services to support adults and seniors, including:

• Senior Case Management: Connecting seniors to community resources, providing emotional support, and facilitating communication between healthcare providers and families.
• Long-Term Case Management: Helping individuals with chronic illnesses or disabilities navigate the healthcare system, access necessary services, and manage their care.

My Approach

My approach is collaborative, tailored to each client's unique needs and goals. I believe in creating a safe and supportive environment where individuals feel empowered to share their experiences, explore their challenges, and develop coping strategies.

Insurance and Fees

I do not participate in-network with any insurance panels. I charge $125 per hour for case management services. I am currently only accepting private pay. Please note that cancellations must be made at least 48 hours in advance to avoid being charged for the session.
